Hospital in Your Home USA (HiYH) is a Black woman-owned, healthcare innovation company that delivers high-touch, high-tech medical care directly into the homes of underserved communities. We specialize in Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) for Medicare and Medicare Advantage beneficiaries, with a strong emphasis on reducing hospitalizations, improving health equity, and empowering patients to age in place. HiYH s expanded Florida model includes: Aging with Grace: A culturally responsive aging-in-place initiative that brings preventive screenings, care coordination, and community-based wellness programs to seniors bridging clinical care with trusted neighborhood engagement. Transitional Care Management (TCM): A hospital-to-home model supporting patients during the critical 30-day period after discharge, helping reduce readmissions through nurse-led home visits, virtual consultations, medication reconciliation, and continuous monitoring. RPM/CCM Services: A tech-enabled chronic disease management program that leverages wearable devices and virtual care to track vital signs, provide early interventions, and support long-term health outcomes.
